It is the format, and it works because it is mutual, one dressed person in the room reintroduces exactly the asymmetry the session exists to remove. That said, nothing here runs on compulsion: start how you are comfortable and the first ten minutes usually settle the question on their own.
It is franker than a spa hour by definition, but the register is still yours to set. Some guests book it purely for the unbroken skin contact and keep the session at that; others let it travel. She reads which evening she is in and answers accordingly.
